彩色多普勒超声对妊高征胎儿宫内窘迫的诊断价值

摘    要:目的探讨彩色多普勒血流显像(CDFI)对妊娠高血压综合征(PIH)患者胎儿宫内缺氧的诊断价值。方法应用CDFI检测78例正常晚期妊娠组和49例PIH胎儿窘迫组胎儿大脑中动脉(MCA)及肾动脉(RA)的血管收缩期峰值速度(S)与舒张末期流速(D)比值(S/D)、搏动指数(PI)及阻力指数(RI),并与妊娠结局相对照。结果胎儿窘迫组MCA阻力指标均低于正常妊娠组(P〈0.01),RA阻力指标明显高于正常妊娠组(P〈0.01),胎儿窘迫组MCA和RA的S/D、PI、RI关系密切,均呈直线负相关(P〈0.01)。宫内窘迫组围产儿结局异常的百分比明显高于正常妊娠组(P〈0.005)。结论宫内窘迫胎儿MCA及RA的阻力指数呈负相关,二者联合检测对预测妊高征胎儿宫内窘迫有重要临床意义。

Abstract:Objective To study the value of color Doppler blood flow imaging in diagnosis of intrauterine fetal hypoxia in women with pregnancy-induced hypertension(PIH) syndrome.Methods Ratio of peak blood flow velocity,pulsation index(PI) and resistance index(RI) were detected by color Doppler blood flow imaging in systolic and diastolic(S/D) middle cerebral artery(MCA) and renal artery(RA) of 78 normal fetuses and 49 intrauterine hypoxia fetuses in women with PIH syndrome,and compared with those at the end of pregnancy.Results The RI of MCA was lower while that of RA was higher in normal fetuses than in those with hypoxia(P0.01).The S/D,PI,and RI of MCA were closely related to those of RA in fetuses with hypoxia(P0.01).The percentage of outcome was significantly higher in perinatal fetuses than in normal fetuses(P0.005).Conclusion The RI of MCA and RA is negatively related in fetuses with hypoxia.Their detection is of great importance in predicting intrauterine fetal hypoxia.
