两种不同方式治疗腰椎体滑脱症的效果观察

目的探讨复位固定（RF）椎弓钉棒系统联合螺纹状椎间融合器（TFC）内外植骨治疗腰椎体滑脱症的临床疗效和并发症情况。方法选择沧州和平医院120例腰椎椎体滑脱症患者，随机选以其中80例为观察组，行RF椎弓根钉棒系统＋TFC内外植骨法治疗；另外40例为对照组，行经后路腰椎椎体间自体髂骨植入融合术治疗，比较两组治疗效果。结果观察组患者临床治疗后总有效率和优良率均明显高于对照组（P〈0．05），观察组术后植骨融合良好，无腰椎椎体滑脱加重或复发病例，只出现1例（1．25％）内固定松动患者，并发症发生率显著低于对照组（P〈0．01）。结论RF椎弓根钉棒系统联合TFC内外植骨治疗腰椎体滑脱具有较好的临床疗效，可提高伤椎复位后的稳定性．减少滑脱加重或复发。

Efficacy research of two different ways in treating lumbar spondylolisthesis

Objective To investigate the clinical effect and complications of RF pedicle screw system fixation with TFC in- side and outside bone graft in treating lumbar spondylolisthesis. Methods 120 cases of patients with lumbar spondylolis- thesis were selected. 80 cases of them were randomly chosen as observation group, who were given the treatment of RF pedicle screw system fixation with TFC interverebral fusion and 40 cases were randomly chosen as control group, who were received posterior approach iliac crest graft for lunbar interbody fusion. The effect of two groups were compared. Results The total effective rate and excellent rate of observation group were significantly higher than the control group （P 〈 0.05）. All of the observation group had good bone fusion and no case of lumbar spondylolisthesis-emphasis or relapse occurred . There was only one case （1.25%） which found with fixation loosening. Compared with control group, the observation group had lower rate of complications （P 〈 0.01）. Conclusion The treatment of RF pedicle screw system fixation with TFC inter- verebral fusion has a better clinical effect in treating lumbar spondylolisthesis, which can stabilize the ipsilateral lumbar af- ter rest and reduce aggravation or relapse.
