老年冠心病患者择期介入治疗对血浆B型钠尿肽的影响

目的探讨血浆B型钠尿肽(BNP)水平与老年冠心病择期PCI患者的关系。方法选择行择期PCI的冠心病患者284例,根据病变分为单支病变组109例、双支病变组99例、多支病变组76例。分别检测患者PCI术前及术后7 d BNP、肌钙蛋白I(cTnI)和LVEF,同时分析PCI术前、术后BNP水平与冠状动脉病变的关系。结果与单支病变组比较,双支和多支病变组患者BNP和cTnI水平升高,3组两两比较,差异有统计学意义(P<0.01)。3组择期PCI术后BNP水平较术前降低,差异均有统计学意义(P<0.01),而择期PCI前后cTnI、LVEF差异无统计学意义(P>0.05)。结论老年冠心病患者行择期PCI可改善心肌缺血,降低BNP水平。BNP水平可作为判断心肌缺血的临床指标之一。

Effect of PCI on plasma brain natriuretic peptide in elderly patients with coronary heart disease

Objective To study the correlation between plasma brain natriuretic peptide(BNP) level and PCI in elderly patients with coronary heart disease(CHD).Methods Two hundred and eighty-four CHD patients undergoing PCI were divided into single lesion group(n= 109),double lesion group(n = 99),and multiple lesion group(n=76) according to the number of involved vessels. Serum levels of BNP,cardiac troponin(cTnI) and left ventricular ejection fraction(LVEF) were measured before and 7 days after PCI.Relation between BNP level and coronary lesion was analyzed.Results The serum BNP and cTnI levels were significantly higher in double and multiple lesion groups than in single lesion group(P<0.01).However,no significant difference was observed in serum cTnI level and LVEF among the 3 groups before and after PCI(P>0.05).Conclusion PCI can improve myocardial ischemia and decrease BNP level in elderly CHD patients. Serum BNP level can be used as one of the clinical indications for the assessment of myocardial ischemia.
