苯妥英治疗氯丙嗪所致抑郁状态

目的观察苯妥英对氯丙嗪所致抑郁状态的疗效。方法采用双盲随机对照研究。研究组（氯丙嗪＋苯妥英）21例，对照组（氯丙嗪＋安慰剂）20例。分别于入组前、治疗第2wk、第4wk用汉米顿抑郁量表（HAMD）、抑郁自评量表（SDS）和副反应量表（TESS）进行评定．结果研究组患者用苯妥英治疗后抑郁量表（HAMD和SDS）评分校治疗前明显降低，特别是焦虑／躯体化、认知障碍和睡眠因子。但对照组治疗前、后抑郁量表评分变化不明显，研究组的TESS评分中抗胆碱能副作用显著增加（＜0.05）。结论苯妥莫能改善氯丙嗪所致的抑郁状态。

Effects of Phenytoin on depressive syndrome induced by chlorpromazine

To observe the phenytoin's effects on depressive syndrome induced by chlorpromazine.Method Double- blind placebo- control study in 4l schizophrenic patients suffered from depressive syndrome after taking chlorpromarine.Twenty- one samples took phenytoin, and 20 samples took from placebo. HAMD, SDS and TESS were observed respectively before treatment and at the end of the forth week.Result The scores were decresed significantly in SDS and HAMD after treatment with phenytom than before treatment, especially in the factors of anxiety / somatization cognitive disturbance, and sleep disturbance. No significant changes of the scores on the above two scales could be found in control group. The anticholibergic effect was higher in phenytoin group.Conclusion Phenytoin can improve schizophrerua's depressive symptoms induced by chlorpromazine.
