贲门失驰缓症的外科治疗

摘    要:目的探讨贲门失驰缓症的手术治疗及其效果。方法贲门失驰缓症患者48倒,行Heller手术15例,Neller氏术加贲门成形术12例,Heller手术加食管胃底固定术21例。术后从症状学、X线上消化道钡餐造影及纤维胃镜检查观察治疗效果。结果食管胸膜瘘死亡1例(208%),39例经6mo~8a随访,术后狭窄再手术2例,痊愈30例(7692%),好转9例(23.08%).而Heller手术加食管胃底固定术21例中,17例经6mo~3a随访体重增加,进食顺利,无反酸及烧心症状。结论Heller氏术加食管胃底固定术在预防胃食管反流方面优于其它术式。

Abstract:To research the operative method and therapeutic effect of achalasia of the cardia.Methods Forty-eight patients with achalasia of the cardia were observed. Fifteen patients were treated by Heler's operation, Heller's operation combined with cardioplasty (12 cases), Heller's Operation com-bined with esophagofundopexy (21 cases). Postopera-tive observed therapeutic effect according from symp-tomatology, swallow barium and upper gastrointestinal radiogrph and the result of fibaptic gastroscopiy.Results One cass (2. 08% ) was dead from esophagopleura fistula. The postoperative follow -upvaried from 6 month to 8 year. 2 cases was performed second opertion because esophagus stenosis, the cure rate was 16. 92 %, the rate of improvement was 23.08 %. Heller's operation combined with esophagopexy in 17 cases, postoperative their weight were rised, and dysphagia, sour reguritation, heartburn hadn' t been found in folliwing up stage from 6 month to 3 year.Conclusions Heller's operation combined with esophagofundopexy is best opoerative methods for acha-lasia of the cardia in preven gastroesphageal reflux.
