胃癌组织中膜型1-基质金属蛋白酶的表达

摘    要:目的 探讨膜型1-金属蛋白酶在胃癌组织的表达以及与胃癌侵袭和转移的关系。方法 采用免疫组织化学SP方法检测了47例胃癌组织MT1-MMP的表达,采用秩和T检测进行临床病理资料分析。结果 MT1-MMP在正常胃组织中几乎不表达(1/15),而在胃癌组织中高表达(42/47)。进展期胃癌表达(41/42)明显高于早期胃癌(1/5)(P<0.01),肠型胃癌(24/28)和弥漫型胃癌(12/12)均高表达MT1-MMP,但弥漫型胃部的表达更显著(P<0.01),浆膜侵袭组表达(32/33)与非浆膜侵袭组(10/14)的差别有非常显著的意义(P<0.01),淋巴结转移组(31/32)与非淋巴结转移组(11/15)的差别有非常显著的意义(P<0.01)。MT1-MMP表达定位在胃癌侵袭生长的边缘区,其中以瘤细胞为主,部分基质细胞也表达。分布形式主要有两种。带状分布主要见于肠型胃癌的粘膜,粘膜下层以及肠型胃癌和弥漫型胃癌的肌层侵袭。弥漫性分布多见于弥漫型胃癌的全层侵袭。结论 MT1-MMP表达促进胃癌的的胃壁侵袭和淋巴结转移,可作为判断胃癌恶性表型有有用指标。

Abstract:Objective To study the expression of membrane type 1 matrix metalloproteinase (MT1 MMP) in human gastric cancer (HGC) and its relationship with invasion and metastasis of GC. Method Using immunochistochemistry the expression of MT1 MMP was determined in 47 cases of surgically resected HGC. Results MT1 MMP was over-expressed in HGC(42/47), while it has hardly found in the normal gastric tissue(1/15). The level of MT1 MMP expression in advanced HGC(41/42) was significantly higher than in early GC(1/5) ( P <0.01). The expression of MT1 MMP in the diffuse HGC (12/12) were higher than in the intenstinal HGC (24/28) ( P <0.01). The positive rates of MT1 MMP expression in the groups with serosal invasion(32/33) and lymph node metastasis (31/32) were much higher than those in the group with negative ones (10/14 and 11/15) ( P <0.01). The expression MT1 MMP was showed in the extracellular matrix of tumor tissues, which surrounded the invasive margin of cancer tissues. The positive cells at these sites were mainly HGC cells. Conclusion There is good correlation between the expression of MT1 MMP and the invasion, metastasis of HGC. So it can be used as a useful marker for invasion and metastasis of HGC.
