红细胞冷凝集对不同类型血细胞分析仪检测结果的影响探讨

摘    要:目的:探讨红细胞冷凝集对不同类型血细胞分析仪检测结果的影响,并提出处理方法。方法:将红细胞冷凝集标本分别在ADVIA-120、SF-3000和BC-3000^PLUS 3类不同仪器上测定,并对37℃水浴前后的结果进行分析比较。结果:水浴前,WBC和RBC分布图均明显异常并有相应的报警;HGB轻度减低,RBC和HCT结果显著减低,MCV、MCH、MCHC和RDW结果均显著增高.WBC和PLT结果在3种仪器上表现不一。水浴后,血细胞分布图形和各参数的检测结果均恢复正常。结论:红细胞冷凝集对不同类型的血细胞分析仪测量的血细胞参数结果均有不同程度的影响.对怀疑为红细胞冷凝集的标本,必须置37℃水浴消除冷凝集后进行重新测定才能报告结果。

Abstract:Objective To investigate the effect of test dimension in the three different type cytoanalyzer for cold agglutination, and put forward solve measures. Methods The specimen of the akaryocyte cold agglutination was determined respectively with the Bayer ADVIA-120, Sysmex SF-3000 and Mindray BC-3000^PLUS and the results were analyzed in 37 ℃ water bath in front and back. Results The cytoanalyzer can be displayed the abnormal scatter and histogram for the white blood cell(WBC) and the red blood cell (RBC), and gave corresponding alarm before of 37 ℃ water bath. Hemoglobin(HGB) is lightly decreased(P〉0.05).RBC and haematocrit(HCT)were significantly decreased(P〈0.001). Whereas the results of mean corpuscular volume(MCV), mean corpuscular hemoglobin(MCH), mean corpuscular hemoglobin concentration(MCHC) and red blood cell volume distribution width(RDW)were significantly increased(P〈0.01). WBC and platelet(PLT) were showed different diversity in the three type cytoanalyzer. They were recovered normally after 37 ℃ water bath. Conclusion The akaryocyte cold agglutination has significantly effect on results of test dimension in the distinguished type cytoanalyzer. Therefore the suspected cold hemagglutination specimen should be put in 37 ℃ water bath to eliminate the effect and measured again.
