目的：探讨十二指肠镜治疗胆道蛔虫病的效果。方法：回顾性分析1981～2002年经十二指肠镜治疗2072例胆道蛔虫病临床资料。结果：119例乳头嵌塞型胆道蛔虫经十二指肠镜取虫，成功率100％(119／119)；1953例胆管腔内型胆道蛔虫单纯十二指肠镜取虫成功率87．1％(1701／1953)，结合：ERCP及EST取虫成功率93．9％(92／98)，总成功率92．5％(1912／2067)。术后急性胆管炎及急性胰腺炎发生率分别为1．3％(26／2072)及1．6％(34／2072)。结论：经十二指肠镜取虫治疗胆道蛔虫病具有安全、准确、成功率高、创伤小的优点，可作为常规治疗技术在蛔虫高发地区推广。

Treatment of 2072 cases of biliary ascarid with duodenoscopy

Objectives To investigate the effect of the treatment of biliary ascarid with duodenoscopy. Methods To analyze the clinical data of 2072 cases of biliary ascarid treated by duodenoscope from 1981 to 2002 retrospectively. Results In 119 cases of duodenal papilla impaction type,the success rate of duodenoscope treatment was 100%(119/119).There were 1953 cases of intrabiliary tract type,the success rate was 92 5%(1912/2067),including success rate of duodenoscope only 87 1%(1701/1953),and that of duodenoscope combined with ERCP and EST 93 9%(92/98).The rate of acute cholangitis and pancreatitis after treatment was 1 3%(26/2072) and 1 6%(34/2072) respectively. Conclusion The duodenoscopy appears to be safe and acurate with high success rate and less traumatic procedure for the treatment of biliary ascarid. Duodenoscopy management could be widely used as a routine therapy for the treatment of ascarid in biliary tract.
