神经节细胞胶质瘤的影像诊断

摘    要:目的:探讨神经节细胞胶质瘤(ganglioglioma,GG)的影像学特点,提高对这一少见脑肿瘤的认识。方法:回顾性分析经病理证实的10例GG的影像学表现。10例中,1例仅行CT平扫,5例行MR平扫+增强扫描(其中1例同时行MR灌注成像(peffusion weighted imaging,PWI),1例同时行PWI及MR波谱成像(MR spectroscopy,MRS)),3例同时行CT及MPd扫描(包括平扫、增强及1例PWI)。结果:10.例中,1例为脑内多发病变,9例单发病变中,5例位于颞叶,2例于小脑,2例于基底节区。所有病变未见明显占位征象,瘤周水肿无或轻微。CT平扫3例中2例呈低密度,1例因病变小且位于左颞叶边缘,CT检查未发现病变;CT增强扫描2例见病变实性部分明显强化;MRI平扫8例病变呈T1WI低信号为主、T2WI高信号为主;DWI表现多样;MRI增强扫描8例呈环形强化、团片状强化、弥漫强化或无明显强化;PWI检查3例未见局部脑血容量(rCBv)升高;MRS检查1例仅显示NAA轻度减低,未见异常代谢产物。结论:GG的临床和影像表现有一定特点,联合应用多种影像学检查方法有助于该病变的术前诊断。

Abstract:Objective:To evaluate the imaging characteristics of ganglioglioma (GG). Methods:10 patients with pathologically proved GG were retrospectively reviewed, including 1 patient with only pre-contrast cr scan, 5 patients with pre-and post-contrast MR scan, 1 patient also tmderwem PWI and MRS, 3 patients with multiple imaging modalities including CT and MR scans. Results: 1 patient had multiple lesions. 9 patients had single lesion, which included 2 lesions in the cerebellum, 5 lesions in the temporal lobe, and 2 lesions in the basal ganglia. No markedly mass effect was found in all cases. No peritumoral edema was found in 8 cases and mild edema in 2 cases. CT scan was negative in one patient because the lesion was small and located at the border of the left temporal lobe. Hypodensity was seen on pre-contrast CT scan. Homogeneous or heterogeneous T1WI hypointensity and T2WI hyperintensity were seen on pre-eontrast MR scan. Peripheral enhancement, clump enhancement, diffuse enhancement or no appreciable enhancement was seen on post-contrast CT or MR scan. Perfusion imaging demonstrated no definite increased relative cerebral blood volume (rCBV) in the region of the mass. Spectro- scopic imaging demonstrated normal spectra within the region of the mass. Condusion: The clinical manifestation and imaging findings of GG are somehow characteristic. It is helpful for preoperative diagnosis of GG to combine multiple imaging modalities.
