曲马多对全身麻醉苏醒期和气管拔管反应的影响

目的:观察全身麻醉苏醒期静脉注射曲马多对全身麻醉苏醒、气管拔管反应的影响。方法:选择ASA Ⅰ～Ⅱ级患者120例,按随时平均分为C组(对照组)、T组(研究组),两组在手术结束前10min分别静脉注射0.9％生理盐水、曲马多(2mg/kg)稀释液5ml,在2min内注射完毕,3min后减浅麻醉,观察比较两组患者的苏醒速度,气管拔管反应及全麻并发症。结果:两组在完成指令性动作和拔除气管导管的时间上无差异;但在拔管时C组患者的心率、血压变化显著高于T组;监测脉搏氧饱和度(SPO2),拔管时即刻C组低于T组,有统计学意义,拔管后C组亦低于T组,但无统计学意义;T组并发症显著少于C组。结论:在全身麻醉苏醒期应用一定剂量曲马多不影响患者苏醒,不抑制呼吸而且可以抑制气管拔管反应并有助于减轻苏醒后疼痛,减少全麻并发症。

The effect of intravenous tramadol on the reaction of tracheal extubation and Anesthesia recovery period

Objective To observe the effect of trmadol on anesthesia recovery period and reaction of tracheal extubation. Method 120 ASA I-II patients were randomly allocated to two groups .About 10 minutes before the end of surgery ,0.9% normal saltine solution (Group C) or tramadol with a dose of 2 mg/kg (Group T) was intravenously infused ,each of which was diluted into 5 ml , and the depth of anesthesia was weakened 3 minutes later .The time of recovery was recorded ,and the reactions of tracheal extubation and the side-effects were also observed. Results In group C ,heart rate and blood pressure was increased significantly compared with that in group T;arterial oxygen saturation was lower severely than in group T at the time of extubation .while 5 minute later , the difference was insignificant .The time recoverying for response to command and for tracheal extubation was not different between two groups .Side-effects were more in group C than in group T. Conclusion Tramadol intravenously infused with a dose of 2 mg/ kg has little effect on respiration and recovery.restrains the reaction of tracheal extubation and releases side-effect of general anesthesia .So it can be used safely and satisfactorily during anesthesia recovery period .
