消化性溃疡患者的心理社会因素及PSG研究

目的 探讨心理社会因素在消化性溃疡(PU)中的作用以及PU患者的多导睡眠图(PSG)模式。方法 采用一般情况调查表、MMPI、HAMA、HAMD以及生活事件量表(LES)、社会支持量表(SS)、多伦多述情障碍量表(TAS)对29例PU患者进行评定，对其中11人进行PSG检查。以30例正常人作对照。结果 PU组与对照组在MMPI的Hs、D、Hy和HAMA、HAMD、TAS差异有显著性，Hp(—)组较Hp( )组的心理社会因素多。PU组存在睡眠进程紊乱，第一个REM睡眠时间及百分比增加。结论 PU患者存在一定的个性特征及情绪障碍，与其发病密切相关。PU存在不同的亚型，部分主要与感染有关，部分主要与心理因素有关。消化性溃疡的多导睡眠图模式与抑郁症有类似的变化趋势，提示这两种疾病之间存在某种内在联系。

The psychosocial factors and polysomnography study of peptic ulcer

Objective To research the psychosocial factors and polysomnography of peptic ulcer(PU).Methods 29 patients with PU were assessed by Scale for the general condition,Minnesota Multiphasic Personality Inventory(MMPI),Hamilton Depression Scale(HAMD),Hamilton Anxiety Scale(HAMA),Toronto Alexithymia Scale(TAS),Life Evevt Scale(LES),Social Support SCale(SS).Polysomnography were performed before receiving medications.The control group consisted of 30 normal Chinese individuals.Results Patients with peptic ulcer had statistically significantly higher scores in scale Hs,D,and Hy of MMPI,and in the three mood scales(HAMA,HAMD,TAS)compared with the controls.There was no significant difference in scale LES and SS between the two groups.The area of ulcer was positively correlated with scores on scale HAMA and TAS.Patients with peptic ulcer exhibited sleep continuity disturbance,i.e.increased awakening time and number,diminished sleep maintenance.Increased First rapid eye movement(REM)sleep time and percent was also noted.Conclusion Patients with peptic ulcer had personality and mood disturbance. 1Patients with peptic ulcer exhibited the similar abnormality of sleep EEG as depression,which showed peptic ulcer and depression maybe have relationship in neurobiological aspects.
