摘    要:目的:探讨急性胰腺炎(AP)患者电解质及血气的变化。方法:将63例AP患者分为轻症组(MAP)和重症组(SAP),分析发病48小时内采集的动脉血气及电解质变化。结果:SAP组的PaCO2、PaO2、HCO3^-明显低于MAP组(P〈0.05),pH和阴离子间隙(AG)两组无显著差别。在酸碱代谢方面,以呼吸性碱中毒或呼吸性碱中毒并代谢性酸中毒最常见,其次为代谢性酸中毒、呼吸性酸中毒并代谢性酸中毒、代谢性碱中毒。电解质紊乱以低血钙为主。结论:AP患者低氧血症、酸碱平衡及电解质紊乱在ARDS、MODS发生前即已存在,低氧血症可能是一种早期的MODS信号,通过进行动脉血气及电解质的监测,进行早期干预,对预防MODS的发生,降低病死率有重要的临床意义。

Abstract:objective:To observe the changes on artery blood gas and electrolyte in patients with acute mild -or - severe - type pancreatitis. Methods: Documents was selected from January 2001 through July 2007 in our hospital and all cases were divided into the mild and severe acute pancreatitis group (MAP and SAP group). The samples had been collected from patients' artery blood in 48 hrs development of pancreatitis. The level of blood gas and electrolyte was compared between the two groups. Results : level of PaO2, PaCO2, and HCO3^- in SAP group were obviously lower than those in MAP group (P 〈0.05 ). No difference in level of PH and anion gap (AG) between MAP and SAP groups. The respiratory alkalsis or respiratory alkaisis with metabolism acidosis were a greater expression in the two groups. A hypocalcemia showed obviously during disorder metabolism. Conclusions: Hypoxia has developed before the occurrence of ARDS and/or MODS in acute pancreatitis patients and detection of artery blood gas and electrolyte are to have a clinical significant for prevention of MODS and dead in acute pancreatitis.
