经皮肾镜取石与输尿管碎石治疗嵌顿性输尿管上段结石临床效果及安全性

目的:探讨经皮肾镜取石术与输尿管碎石术治疗嵌顿性输尿管上段结石临床疗效及安全性分析。方法:选取我院泌尿外科收治的146例嵌顿性输尿管上段结石患者,按照手术方式的不同将患者分为观察组和对照组,其中观察组(79例)采用经皮肾镜取石术治疗,对照组(67例)采用输尿管碎石术治疗,对比两组患者的手术时间、结石清除率、住院时间及并发症发生率。结果:观察组患者在平均手术时间、结石清除率上明显优于对照组,两组差异具有统计学意义(P<0.05);两组患者在住院时间及并发症发生率无明显统计学差异,无统计学意义(P>0.05)。结论:与输尿管碎石术相比,经皮肾镜取石术治疗嵌顿性输尿管上段结石良好的临床疗效,且手术风险不高,值得临床推广使用。

EFFECT AND SECURITY OF PERCUTANEOUS NEPHROLITHOTOMY AND URETEROSCOPIC LITHOTRIPSY FOR THE TREATMENT OF INPACTED UPPER URETERAL STONES

Objective：To observe and compare therapeutic effect and security of percutaneous nephrolithotomy and ureteroscopic lithotripsy for the treatment of impacted upper ureteral stones. Methods： 146 patients with impacted upper ureteral stones were chosen in our hospital ,all patients randomly divided into control group （67 patients）and experimental group （79 patients）. After treatment the operative time, hospital staying time, the hemoglobin decreased degree, the stone clearance rate and complication incidence were com- pared. Results：The operative time and stone clearance rate of experimental group were significantly higher than control group （P 〈 0. 05 ）. There was no significant difference in hospital staying time and complication incidence between two groups . Conclusion： The percutaneous nephrolithotomy on impacted upper ureteral stones obtains excellent effect and can effectively improve the stone clearance rate, has no serious complication and is worthy of clinical application.
