微创与开腹直肠癌根治术的临床对比分析

目的:对比分析腹腔镜与传统开腹直肠癌根治术在直肠癌治疗中的应用。方法:回顾分析2006-06～2011-12期间218例腹腔镜直肠癌根治术及127例开腹直肠癌手术的临床资料,并针对手术方法、疗效、并发症等方面进行总结评价。结果:腹腔镜手术组的术中出血量、肠功能恢复时间、住院时间、标本远切端长度指标明显优于传统开腹手术组,差异有统计学意义(P<0.05);而手术时间、术中及术后的近期并发症、淋巴结清扫数目、标本近切端长度、肿瘤局部复发及远处转移率两组差异没有统计学意义(P>0.05)。结论:腹腔镜直肠癌根治术具有安全、可行、创伤小、康复快的优点,符合肿瘤根治原则,值得临床推广应用。

A COMPARATIVE CLINICAL ANALYSIS OF LAPAROSCOPIC AND OPEN RADICAL OPERATION FOR PATIENTS WITH RECTAL CANCER

Objective： To compare the results of laparoscopic and open radical operation in the management of rectal cancer. Methods：The clinical data of 218 cases with rectal cancer treated with laparoscopic radical operation and 127 cases treated with open radical operation from June 2006 to December 2011 were analyzed retrospectively. Results： The difference in mean bleeding volum,intestinal function recovery time, duration of hospitalization and specimen far cut end length was significant between these two groups （ P 〈 0.05 ）, but the difference in operating time, peri - operative and postoperative complications, the number of lymph node dis- section,specimen nearly cut end length, tumor local recurrence rate and distant metastasis rate was not statistically significant （ P 〉 0.05 ）. Conclusion： Laparoscopic radical resection of rectal cancer is safe , feasibile, with less trauma and more rapid recovery. It is in conformity with the surgical oncologic principles and deserves promotion and wide application.
