体位护理在小儿呼衰机械通气中的应用

摘    要:目的 观察体位变更在小儿呼衰机械通气治疗中应用的效果 ,并探讨其机理和护理。方法 将 32例呼衰患儿随机分成实验组和对照组 ,实验组 2 0例在机械通气时行体位变更 ,每4h按仰卧位、侧卧位、俯卧位、仰卧位顺序变换。对照组 12例行常规持续仰卧位机械通气 ,2组均在通气后 1,5 ,9,13h ,做血气分析 ,观察 2组患儿相同时间段的氧合状态、血流动力学、并发症等。结果 试验组PaO2 和PaO2 /FiO2 高于对照组 ,2组血流动力学指标对比差异无显著性。结论 患儿呼衰机械通气中变更体位与传统仰卧位比较 ,可有效提高氧合状态和治疗效果 ,预防并发症。

Abstract:Objective To observe the application of posture changing for pediatric respiration exhaustion was treated by mechanical respiration and explore its mechanism and nursing. Methods Thirty-two patients with pediatric respiration exhaustion were randomly divided into experimental group and control group.The position was changed for the twenty patient of experiment group then mechanical management of respiration in turn supine position,laleral decubitus,prone position,supine position every 4 hours.Twelve patients of control group were regularly treated by mechanical management of respination in continuous supine position.The blood gas were analyzed after mechanical management of respiration in turn 1 hour,5 hour,9 hour,13 hour respectively. Oxyhemostate,hematodynamics and complications were observed at synchrotime. Results PaO 2/FiO 2 of experiment group are higher than control group.There was no significant differences between 2 groups for the hematodynamics exponent. Conclusions Compared with the roution supine position oxyhemostate and treating effect are improved,complication can be prevented.
