22例婴儿巨细胞病毒感染的临床分析

目的 探讨婴儿巨细胞病毒感染的临床特点、检测手段及治疗。方法 对22例经尿CMV－DNA PCR及血清CMV－IgM ELISA测定确诊为巨细胞病毒感染的婴儿患者的临床资料进行分析。结果 尿CMV－DNA PCR阳性14例，血清CMV－IgM ELISA阳性22例，两者均阳性9例，患儿母血清CMV－IgM阳性20例。肝肿大19例，黄疸15例，脾肿大10例，咳嗽、气促8例，抽搐1例，小头畸形1例。所有的病例均应用更昔洛韦治疗。结论 巨细胞病毒感染以肝脏损害最多见，CMV－感染的母亲是小儿CNV感染的重要来源。用PCR检测尿中CMV－DNA和用EUSA检测血清中CMV－IgM是诊断CMV感染的常用方法。更昔洛韦治疗CMV感染效果好。

Diagnosis of treatment of 22 infant cases with cytomegalovitus

Objective To explore the clinical characteristic of infant infected with cytomegalovirus (IIC) discussion the detective and methods of infant infected with cytomegalovirus (CMV).Methods The clinical material of 22 cases of IIC, which diagnosed by Urine CMV-DNA through PCR and serum CMV-IgM used ELISA technic, were analyzed.Results Urine CMV-DNA of 14 cases were assayed to be positive, serum CMV-IgM of 22 cases were detected to be positive, 9 cases of both above assay were positive, and 20 out of the 22 cases of IIC mother were positive in serum CMV-IgM assay There were 19 cases with megalohepatia, 15 cases had jaundice, 10 cases with splenomegaly, 8 cases had cough and breathless, 1 case convulsion, 1 case microcephaly. All the cases were treated by ganciclovir.Conclusion The most common symptom in CMV infection was liver injury. The infection from mother was a important cause of IIC PCR is used to detect urine CMV-DNA and semm CMV-IgM, assaged by ELISA are the usual methods of diagnose of CMV infection. PCR is more sensitive. The treatment of CMV infection with ganciclovir showed a good effect.
