| 老年非霍奇金淋巴瘤继发急性肿瘤溶解综合征的临床病理学特征 |

| 摘 要： | 本研究旨在提高对老年恶性淋巴瘤患者发生急性肿瘤溶解综合征(acute tum or lysis syndrom e,ATLS)的临床病理特征的认识,探讨其诊断和治疗方法。通过回顾性分析2007年本科收治的两例恶性淋巴瘤老年患者发生ATLS前肿瘤的进展情况,以及发生ATLS后的临床表现、实验室检查特点和治疗反应等临床资料,探讨ATLS的临床病理特征。结果表明,2例高危非霍奇金淋巴瘤(弥漫大B细胞型)患者发病前肿瘤负荷高,对治疗敏感,并存多项ATLS促发因素。发生ATLS后1例表现为低钾血症,1例表现为高钾血症。对2例患者均采用别嘌呤醇、水化、碱化尿液治疗,1例同时接受透析治疗,但2例最终因病情持续进展而死亡。非霍奇金淋巴瘤(弥漫大B细胞型)经利妥昔单抗单药治疗后很少发生ATLS,但若存在多项促发因素也可发生,一般在治疗后1-4天内发生。结论:ATLS是高危恶性淋巴瘤患者死亡原因之一,尤其在老年患者,临床表现不典型,诊断主要依靠实验室检查。针对高危ATLS患者的早期预防、早期诊断和及时治疗对于改善预后有重要意义。
