医院感染的金黄色葡萄球菌耐药性与耐药基因检测

摘    要:目的研究医院感染的金黄色葡萄球菌的耐药性与耐药基因状况,指导临床合理用药。方法对本院感染标本分离出的120例金黄色葡萄球菌(SAU)进行药敏试验,多重PCR法检测aph(2")、aph(3)’Ⅲ、ant(4’,4)"、TEM、tetM、macA、erm和qacA/B基因。结果 120株SAU中,耐甲氧西林金黄色葡萄球菌(MRSA)有71株,占59.1%;甲氧西林敏感金黄色葡萄球菌(MSSA)有49株(40.9%)。MSSA对15种抗生素制药率超过50%的只有6种,而MRSA对15种抗生素耐药率超过50%的12种;经8种基因进行检测,MRSA与MSSA对8种耐药基因携带率各不相同,MRSA高于MSSA。结论医院感染的金黄色葡萄球菌对抗菌药物有很高的耐药性,MRSA较MSSA的耐药性更高、更复杂,必须加强金黄色葡萄球菌的耐药性监控,以最大限度降低其耐药性的发生。

Abstract:Objective to study the hospital infection of staphylococcus aureus resistance and resistance genes,guide clinical rational drug use.Methods the infection specimens from 120 cases of staphylococcus aureus(SAU susceptibility test),multiple PCR method to detect aph(2),(3)-aph Ⅲ,"ant""and"(4),TEM,tetM,macA,erm and qacA/B genes.Results 120 SAU strains resistant,dermatologists(MRSA)have 71 strains,59.1%,Armor oxygen cylindriod sensitive staphylococcus aureus(49),MSSA for 40.9%.MSSA on 15 antibiotic resistance of more than 50%.and only six of 15 antibiotic resistance of MRSA more than 50%of the12,The gene detection.and MSSA MRSA eight kinds of drug-resistant genes carry each are not identical,MSSA higher MRSA.Conclusion the hospital infection of staphylococcus aureus of antimicrobial resistance,high resistance to higher than MSSA MRSA,more complex,must strengthen the monitoring of staphylococcus aureus;reslstance to minimize its reslstance.
