机械通气患者的不同撤机方式的研究

目的通过对不同撤机方式的比较,探讨更好的撤机方式。方法 102例达到撤机指标的患者随机被分成SIMV＋PSV、PSV、T-piece实验3组（每组n=34例）。监测3种不同撤机方式的撤机时间和撤机失败率;监测不同疾病的撤机时间。结果 SIMV＋PSV组平均撤机时间是7.2d;PSV组平均撤机时间是4.8d;T-piece组平均撤机时间是4.7d;SIMV＋PSV组与PSV组和T-piece组的平均撤机时间比较差异均有统计学意义（P〈0.05）;PSV组与T-piece组平均撤机时间比较差异无统计学意义（P〉0.05）。从撤机失败率分析,SIMV＋PSV为29.4%,PSV为14.7%,T-piece组44.1%;而SIMV＋PSV组与PSV组和T-piece组比较差异均无显著性（P〉0.05）;PSV组与T-piece组比较差异有统计学意义（P〈0.05）。从不同疾病的平均撤机时间分析AECOPD和多发创伤的患者平均撤机时间相对较长分别为6.3d和8.5d,而重症哮喘和重症肺炎患者平均撤机时间相对较短分别为2.9d和3.6d。结论 3种撤机方式比较PSV优于SIMV＋PSV和T-piece实验。

The research of different weaning modes in mechanical ventilation

Objective By way of comparison of different weaning modes to explore the better one.Methods 102 cases to target weaning patients were randomly divided into SIMV＋PSV,PSV,T-piece trail three groups（n=34）.Monitoring weaning time and failure rates of the three weaning modes;monitoring weaning time of different diseases.Results SIMV＋PSV group,the average weaning time was 7.2 days;PSV group,the average weaning time was 4.8 days;T-piece group,the average weaning time was 4.7 days;SIMV＋PSV group and the PSV group and the T-piece group,the average weaning time difference was significant（P0.05）;PSV group and the T-piece group,the average weaning time difference was not significant（P 0.05）.From weaning failure rate analysis,SIMV＋PSV was 29.4%,PSV was 14.7%,T-piece group 44.1%;and SIMV＋PSV group with PSV group and T-piece group,there was no significant difference（P 0.05）;PSV group and the T-piece group difference was significant（P0.05）.From different disease analysis,the average weaning time of AECOPD and multiple trauma patients with an average weaning relatively long time were 6.3 days and 8.5 days.While patients with severe pneumonia and severe asthma,the average weaning time is relatively short at 2.9 days and 3.6days.Conclusion Compared in three ways of weaning modes,PSV is superior to SIMV＋PSV and T-piece trial.
