术中低温冲洗降低腔镜甲状腺手术后创面渗出及疼痛的效果观察

摘    要:目的:探讨术中低温冲洗对腔镜甲状腺手术后创面渗出、颈部肿胀、疼痛、手术时间及喉返神经损伤的影响。方法腔镜下甲状腺次全切除术后61例患者随机分为两组:对照组31例,观察组30例。观察组术中采用8-12℃生理盐水局部冲洗创面。对照组不冲洗。分别观察两组患者术后引流量、颈部肿胀程度、疼痛、手术时间及喉返神经损伤的差异,并进行统计学分析比较。结果两组术后引流量、颈部肿胀程度、疼痛等差异均具有统计学意义(P<0.05)。两组患者手术时间、喉返神经损伤差异均无统计学意义。结论术中低温冲洗可有效控制或减轻腔镜手术后渗液渗血、组织肿胀和伤口疼痛。

Effect of Hypothermia Rinse in Operation in Reducing Surface Exudation and Pain after Endoscopic Thyroid Surgery

Abstract:Objective To discuss the effect of hypothermia rinse in operation in reducing the surface exudation, neck swelling, pain, operation time length and recurrent laryngeal nerve injury after endoscopic thyroid surgery. Methods 61 cases of endoscopic sub-total thyroidectomy were randomly divided into 2 groups:observation group (n=30) and control group (n=31); normal saline at the tempera-ture of 8℃to 12℃was used in local rinse of the wound surface while no rinse was made in control group;the difference in postopera-tive drainage volume, neck swelling, pain, operation time length and recurrent laryngeal nerve injury between the 2 groups was observed and statistically analyzed. Results There existed a difference with statistical significance in postoperative drainage volume, neck swell-ing and pain between the 2 groups (P〈0.05) while the difference in operation time length and recurrent laryngeal nerve injury between the 2 groups was of no statistical significance. Conclusions Hypothermia rinse in operation can effectively reduce the exudation, tissue swelling and incision pain after endoscopic thyroid surgery.
