21例青年人胃癌的临床病理特点分析

目的探讨青年人胃癌的临床病理特点,提高对青年人胃癌的早期诊断率。方法收集经病理确诊的21例青年人（≤44岁）胃癌的病例资料,回顾性分析其临床病理特点,包括患者的性别、年龄、幽门螺杆菌感染率、临床表现、病变部位、病理大体类型、病理组织学类型、淋巴结转移率、临床分期、误诊情况等。结果 21例青年人胃癌患者女性多于男性；幽门螺杆菌（Helicobacter pylori,Hp）感染率为76.2%；好发部位是胃窦；进展期胃癌18例（85.7%）；大体类型以BorrmannⅢ型最多见；病理组织学类型以低分化腺癌和黏液腺癌为主；临床分期多为Ⅲ期；81.0%的高淋巴结转移率,误诊率偏高。结论青年人胃癌的临床病理特点是女性多发,Hp感染率高,分化低,恶性程度高,淋巴结转移率高,误诊率偏高。病理检查是确诊胃癌的最可靠方法。

Analysis of Clinicopathological Characteristics of Gastric Cancer in 21 Young Patients

Objective To explore the clinicopathological characteristics of gastric cancer in young people so as to raise the diagnosis. Methods A retrospective analysis was made to the clinical data of 21 young patients（≤44years old）with gastric cancer confirmed by pathological study, including sex, age, the rate of helicobacter pylori（Hp）infection, the lesion location, pathological type, histopathologic type, the rate of lymph node metastasis, clinical stage and misdiagnosis. Results Female were more than male in the 21 young patients with gastric cancer; the Hp infection rate was 76.2%; gastric antrum was the predilenction site of gastric cancer; 18 cases were at progressing stage（85.7%）; Borrmann III was the main pathological type while poorly differentiated adenocarcinoma and mucinous adenocarcinoma were the main histopathological types; most cases were at clinical stage III; the rate of lymph node metastasis was 81.0% and misdiagnosis rate was high. Conclusions Gastric cancer in young people is of such clinicopathological characteristics as high occurrence in female, high rate of Hp infection, poor differentiation, high malignancy, high rate of lymph node metastasis and misdiagnosis; pathological examination is the most liable in diagnosing gastric cancer.
