冷循环射频治疗在胸部实体肿瘤治疗中的应用

摘    要:目的 研究冷循环射频在治疗胸部实体肿瘤中的临床应用价值. 资料与方法 23例胸部肿瘤患者分别在CT导引或手术直视下,分别以单极或多极穿刺针穿刺瘤体,分点一次或多次对肿瘤实体行射频治疗.术中观察射频前后瘤体CT值及邻近组织改变情况,术后观察并发症发生情况,1个月后复查CT了解肿瘤大小变化. 结果 23例实体瘤患者中,显效15例(缩小>原瘤体1/2),有效7例(缩小<原瘤体1/2或无改变),无效1例(瘤体增大或临床症状恶化).23例射频治疗中4例出现气胸,4例出现少量胸腔积液,18例患者出现一过性病灶区疼痛及发热. 结论 冷循环射频治疗在消除胸部小的实体瘤及大的实体瘤减张方面疗效确切,且可反复操作,安全可靠.

Abstract:Objective To evaluate the clinic values of the cool-tip radiofrequency ablation in the treatment of thoracic neoplasm. Materials and Methods Radiofrequency ablation were performed in 23 cases with thoracic neoplasm. Single or multiple cool-tip radiofrequency ablation electrodes were placed percutaneouesly under the CT guiding or thoracotomy. The variations of the CT values within the tumor mass and the adjacent tissues were observed,complications of the RF were also observed after the operations. CT scan was also repeated 1 month after the operation to define the variations of the tumor mass. Results Among the 23 cases with thoracic neoplasm, markedly effective was in 15 cases (65. 2%, tumor size decrease >1/2 of the original mass size), partial effective was in 7 cases(30.4%, tumor size decrease <1/2 of the original mass size or no change of the mass size) , no response of the ablation were 1 case(4.3%). Complications included procedure-related pneumothorax in 4 of 21 percutaneous procedures (19.1%), small pleural effusion in 4 of 21 percutaneous procedures (9/18), transient pain of the focus and fever took place in most patients. Conclusion The response effect of cool-tip radiofrequncy ablation is credible in the treatment of thoracic neoplasm with small and big masses. The operation is relative safety and it can be performed reduplicative in the patients.
