血管内介入治疗蛛网膜下腔出血早发性脑血管痉挛对预后影响的研究

目的:探讨血管内介入治疗早发性脑血管痉挛(CVS),以降低迟发性脑血管痉挛,降低蛛网膜下腔出血的病死率,改善蛛网膜下腔出血的预后。方法:71例经DSA证实有动脉瘤及早发性CVS,全麻下行动脉瘤GDC栓塞后,随机分为两组,治疗组38例采用微导管对载瘤痉挛动脉缓慢灌注尼莫地平针剂0.5mg;对照组33例给予除治疗组尼莫地平以外的相同治疗方法。结果:治疗组的神经功能恢复、迟发性CVS发生率、头痛缓解、病死率、并发症、平均住院日等方面与对照组相比有显著差异。结论:对蛛网膜下腔出血早发性CVS积极地进行血管内介入治疗,可显著改善预后。

Study of influence about prognosis after interventional therapy of intra-blood vessel SAH hageearly onset cerebral vasospasm

Objective:To reduce case fatality ratio of tardive vasospasm and SAH,as well as to improve prognosis of SAH through probe interventional therapy of onset cerebral vasospasm.Methods:71 cases who have been verified to have aneurysm and early onset CVS are devide two groups at radom after aneurysm GDC embolism under the condition of full anaesthesia.One group which is called therapeutics group has 38 cases.All the cases are treated with infuse Nimodipine about 0.5mg slowly into spasmodic artery which carries tumour by adoptin microtubular.Another group called control group has 33 cases which are recepted with the same methods with therapeutics group except Nimodipine.Results:Nervousness function of therapeutics group have been recovered.incidence rate of tardive CVS,headache relif,case fatality rate,complication and average day in hospital and so on have significant deviation.Conclusion:Intervention therapy of intra-blood vessel SAH hageearly onset actively can improve prognosis of patients significantly.
