腹腔镜下多脏器联合手术的临床应用

目的应用腹腔镜技术行腹腔内多脏器多科室联合手术。方法分析该院近6年来由肝胆外科和妇产科联合完成的32例腹腔镜下多脏器手术，其中行胆囊切除术＋胆总管探查取石术＋子宫次全切除术3例，胆囊切除术＋肝囊肿开窗引流术＋子宫次全切除术3例，胆囊切除术＋子宫次全切除术或腹腔镜辅助阴式子宫全切除术15例。胆囊切除术＋卵巢囊肿及畸胎瘤剥除术11例。结果32例手术均顺利完成，手术时间最短40min，最长180min，平均120min，术后平均住院5d，无并发症，效果满意。结论腹腔镜下多脏器多科室联合手术不增加病人痛苦。同时行多个病变部位手术，避免上下腹同时切开或多次手术，住院时间短，费用低。美容效果好。

Clinical application of combined laparoscopic multi-organ surgery

[Objective] To investigate the chnical value and application effect of combined laparoscopic multiorgan surgery. [Methods] 32 combined laparoscopic multi-organ surgeries including laparoscopic cholecystectomy, laparoscopic common bile duct exploration, laparoscopic hepatocystotomy, laparoscopic supracervical hysterectomy, laparoscopic-assisted vaginal hysterectomy, ovarian cyst and benign teratoma extraction were performed from May 1999 to October 2004 by surgeons in Department of Obstetrics and Gynecology and Department of Hepatobiliary Surgery. [Results] All laparoscopic procedures were successfully completed. The operation time ranged from 40 minutes to 180 minutes. And the average operation time was 120 minutes. Mean postoperative hospitalization was 5 days. No complications were found. [Conclusions] Combined laparoscopic multi-organ surgery can be performed conveniently with co-operation of different professional surgeons. It offers significant benefits to patients such as decreased postoperative pain, reduced trauma to the abdominal wall and cost saving.
