放射性核素脑血流负荷显像的临床应用

摘    要:目的:了解放射性核素脑血流负荷显像的临床意义。方法:以乙酰唑胺、双嘧达莫或直立负荷作为介入手段,施行115例次隔日法脑负荷血流灌注显像。通过负荷、静息影像和半定量数据对比,获得脑血管储备功能资料。结果:大脑皮层对负荷介入的局部反应表现为5种类型,可反映其脑血管储备能力,使脑动脉硬化患者的诊断阳性率由静息态的51.6%提高至82.3%,短暂性脑缺血发作(TIA)的诊断阳性率从静息态的45.8%提高至75.0%。结论:脑负荷显像有助于发现隐匿性病灶,提高检出脑血管病变的灵敏度。

Abstract:Objective To evaluate the clinical usefulness and significence of the radionuclide stress brain imaging. Methods The data of 115 cases were obtained through cerebral blood flow stress perfusion imaging with acetazolamide , dipyridamole or upright stress test, and by comparing the images and quantitative data under rest and under stress, and the acquired reserved function of the cerebral vasculature. Results Cerebral cortical response to the stress factors showed 5 patterns. It could be helpful to ensure the reserve ability of the cerebral vasculature. The positive diagnosis of patients with cerebral arteriosclerosis increased from 51.6% under resting state to 82.3% under stress. Likewise, the positive diagnosis of transient ischemic attack increased from 45.8% under resting state to 75.0% under stress. Conclusions The cerebral flow stress imaging is helpful in detecting the occult foci and increases the sensitivity of detection of cerebral vascular lesion.
