示指背皮瓣联合拇指桡侧筋膜皮瓣修复拇指末节缺损

目的 探讨示指背皮瓣联合拇指桡侧筋膜皮瓣修复拇指末节缺损的方法和临床疗效.方法 对12例拇指末节缺损的患者,用示指背皮瓣联合拇指桡侧筋膜皮瓣进行修复,示指背皮瓣切取面积在3.5 cm×3.0cm～5.0 cm×4.0 cm之间,拇指桡侧筋膜皮瓣面积在2.5cm×2.0cm～3.5×3.0cm之间,其中拇指桡侧筋膜皮瓣内的感觉神经均与拇指断端内的指固有神经相吻合,示指背皮瓣修复拇指尺掌侧,拇指桡侧筋膜皮瓣修复拇指桡背侧.结果 12例患者术后均获得成功,经过2～3年的随访,患者对拇指的外形、功能满意.结论示指背皮瓣联合拇指桡侧筋膜皮瓣修复拇指末节缺损的术式简便,可重建拇指的感觉、运动,不牺牲足趾,是修复拇指末节缺损的一种可行性方法.

Distal thumb defect reconstruction using dorsal indicis flap combine with thumb radial fascial flap

【Objective】To introduce a method of using dosal indicis flap combine with thumb radial fascial flap to repair distal thumb tissue defect and clinical results.【Methods】Dorsal indicis flap combine with thumb radial fascial flap were used to repair 12 cases of distal thumb tissue defect.The dimension of the dorsal indicis flap ranged from 3.5×3.0 cm to ～5.0×4.0 cm,and the thumb radial fascial flap ranged from 2.5×2.0 cm to 3.5×3.0 cm.【Results】All the flapd surrvived.After 2 years to 3 years following-up,the patients were satisfied with the flaps appearance and the function of thumb.【Conclusion】It is a reliable approach for the reconstruction of distal thumb tissue defect using dorsal indicis flap combine with thumb radial fascial flap,the advantages include simply procedures,possibility of sensory recovery without sacrificing toes.
