胸部手术后急性呼吸衰竭23例

目的　总结普通胸外科手术后发生急性呼吸衰竭的有关因素和治疗体会。方法　对 2 3例胸部手术后急性呼吸衰竭病人的诊断和治疗进行回顾分析。结果　术前合并糖尿病 6例 ,冠心病 3例 ,慢性阻塞性肺病 4例 ,术后合并食管吻合口瘘 2例 ,支气管瘘 1例 ,肺部感染 5例 ,术中及术后肺水肿 4例 ,均予机械通气 (平均 4.82d) ,死亡 4例 (占 17 4%)。结论　胸部手术后发生急性呼吸衰竭多有明确诱因及术后并发症。早期有效地机械通气和得当地处理合并症是治疗急性呼吸衰竭的关键

Twenty-three Cases of Acute Respiratory Failure after Thorac Operation

Objective To investigate the prevention and treatments of acute respiratory failure after thorac operation.Methods 23 patients with acute respiratory failure after thorac operation were studied retrospectively.Results Complications pro operation are followed:diabetis 6 cases,coronary heartdisease 3 cases and chronic obstructive pulmonary disease 4 cases.Complications post operation are followed:esophagus anatomic leak 2 cases,branch leak 1 case lung infection 5 cases and acute lung edema 4 cases.All patients required mechanical ventilation(mean=4.82 days).4 patients died(mortality=17.4%).Conclusion There always precautions and complications in acute respirtory failure after thorac operation.The key methods are to treat complications and correct application of mechanical ventilation.
