彩超在妊娠高血压综合征孕妇及其胎儿诊治中的应用

目的应用彩色多普勒超声检测妊娠高血压综合征（PIH）孕妇及其胎儿的肾动脉血流变化。方法应用彩色多普勒超声检测120例正常孕妇及其胎儿和49例PIH孕妇及其胎儿肾动脉收缩期血流峰值流速（Vmax）、舒张末期血流流速（Vmim）、阻力指数（RI）。结果轻、中、重度PIH孕妇的肾动脉RI、Vmax、Vmin明显高于正常妊娠组（P〈0．01），中、重度PIH孕妇的胎儿肾动脉RI、Vmax、Vmin明显高于正常妊娠组，且RI增高程度与病情轻重有关（P〈0．01）。结论彩色多普勒超声能动态追踪观察PIH孕妇及其胎儿的肾动脉血流变化，预测PIH孕妇状况及其胎儿宫内状态具有一定的临床应用价值。

Appilcation of Diagnosis and Treatment Progress in Pregnancy Induced Hypertension Gravida and Fetus by Color Doppler Ultrasound

Objective To investigate renal artery hemodynamics of pregnancy induced hypertension(PIH) patients and fetus in various degrees by color Doppler ultrasound.Methods The peak-systolic velocity(Vmax),end-diastolic velocity(Vmin),resistance index(RI) of renal artery.Renal artery of 49 PIH patients and fetus were examined by color Doppler ultrasound,and compared with 120 normal pregnant women and fetus.Results The RI and Vmax of renal artery in PIH patients groups(including mild,moderate and severe PIH groups)were significantly higher than those in normal controls(P<0.01).RI and Vmax of the fetal renal artery in moderate and severe PIH groups were significantly higher than those in normal controls(P<0.01).The change of RI were correlated with disease activity.RI in severe PIH group were significantly higher than those in other groups.Conclusion Color Doppler ultrasonic examination can accurately monitor renal artery hemodynamics of patients and fetus in PIH.It is a useful tool for the observation of stage,evaluation of prognosis and monitor of treatment in PIH patients in order to reducing the death of PIH patients and fetus and improving the perinatal outcome.
