辛伐他汀40mg/d对国人调脂作用有效性和耐受性的Meta分析

目的:评价大剂量辛伐他汀40mg/d对国人调脂作用的有效性和安全性。方法:从中国生物医学数据库(CBMdisc)、中国期刊网全文数据库、维普全文电子期刊和OVID中检索2001年至2007年的相关文献,主要入选标准:(1)研究内容为国内辛伐他汀40mg/d的临床应用;(2)研究对象为心血管疾病、糖尿病、高脂血症病人;(3)研究观察时间不少于2个月;(4)分别记录用药前后血脂的变化及样本量的大小;(5)数据为计量资料。比较应用辛伐他汀40mg/d前后血脂的变化,并利用Rev-Man4.2对数据进行Meta分析,同时对其安全性作出评价。结果:11项研究共517例病人入选,经辛伐他汀40mg/d治疗2个月后LDL-C平均降低1.54mmol/L,TC平均降低2.25mmol/L,HDL-C平均升高0.16mmol/L(P均<0.00001)。主要不良反应为恶心、纳差、腹胀腹痛,共15例;轻度丙氨酸氨基转换酶(ALT)升高11例、轻度肌酸激酶(CK)升高8例,肌肉酸痛2例。均未出现严重的不良反应。结论:辛伐他汀40mg/d在国内的临床调脂治疗中疗效和安全性均较好。

Efficiency and safety of simvastatin 40 mg/d treatment in Chinese people:a meta analysis

Objective:To evaluate the efficiency and safety of simvastatin 40mg/d treatment in Chinese people.Methods:Trials were identified through searching of CBMdisc,CNKI,Weipu and OVID(from year 2001 to 2007) and hand-searching of cross references from original articles and reviews.The major criteria for inclusion were:(1) Clinical trials of simvastatin 40mg/d in Chinese.(2) Patients with cardiovascular disease or diabetes mellitus or hyperlipoidemia.(3) Lasting for at least 2 months.(4) Providing the detail data of the simvastatin therapy(both before and after).(5) The outcomes were putted in measurement data.Then compared the level of cholesterol before and after the therapy and conducted a meta analysis with Revman 4.2. Results:A totel of 11 trials(n=517) were eligible.Averagely,LDL-C were reduced 1.54mmol/L,TC were reduced 2.25mmol/L,HDL-C were increased 0.16mmol/L(P<0.00001).Main adverse effects include nausea,abdominal pain,diarrhea(n=15),slightly increase in ALT(n=11),in CK(n=8) and myosalgia(n=2).No severe side effects had occurred.Conclusion:Simvastatin 40mg/d can effectively lower the level of LDL-C,TC and increase HDL-C in Chinese with good safety.
