内镜下氩等离子凝固治疗食管黏膜不典型增生的临床研究

目的 探讨内镜下氩等离子凝固治疗食管黏膜不典型增生的临床疗效。方法 对32例病理证实为食管鳞状上皮Ⅱ、Ⅲ级不典型增生病灶，在内镜下以氩等离子凝固术完全毁损病变黏膜。从末次治疗开始进行为期3～15个月的内镜随访监测，每次内镜检查时对病变黏膜行色素染色，对可疑病灶进行针对性的活检。结果 对32例食管黏膜不典型增生病灶共进行了52次内镜下氩等离子凝固治疗，每例病灶平均治疗次数为1．625次，治疗成功率为100％，无并发症发生。内镜复查3～15个月无复发。结论 氩等离子凝固术可简便、安全、有效的治疗食管黏膜不典型增生，具有广泛的应用前景。

Endoscopic argon plasma coagulation for esophageal dysplasia

Objective To evaluate the efficacy of endoscopic argon plasma coagulation (APC) for the treatment of esophageal dysplsaia. Methods Thirty-two outpatients were diagnosed as esophageal dysplasia, focuses (0.5-3.5 cm~ 2 in size) at superior esophagus in 7 cases, at middle in 15 cases and at inferior in 10 cases. All patients received APC to completely ablate the lesion. After the last APC, all patients underwent further surveillance endoscopy. At each surveillance, chromoendoscopy was used and the tissues for biopsy were taken from any endoscopically suspicious areas. Results All 32 patients underwent 52 procedures and were all cured. No complications occurred during the procedures. There was no relapse during the follow-up period of 3-15 months. Conclusion APC is safe, simple and effective for the treatment of esophageal dysplasia.
