下呼吸道感染阴沟肠杆菌流行及耐药性分析

摘    要:目的 调查下呼吸道感染阴沟肠杆菌的流行情况及耐药现状,为临床治疗阴沟肠杆菌的感染提供依据.方法 用VITEK自动化细菌鉴定及药敏分析系统对2008年1月-2010年12月住院患者痰液标本中分离的菌种进行鉴定及药敏,对检出的阴沟肠杆菌的药敏结果进行统计分析.结果 2008、2009、2010年全院阴沟肠杆菌菌株所占比例分别为2.5%、2.4%、1.5%,神经外科、泌尿内科的检出率明显高于全院平均水平;阴沟肠杆菌对亚胺培南、美罗培南未出现耐药菌株,对阿米卡星、头孢吡肟的耐药率较低,分别为14.0%、27.1%,其他抗菌药物耐药率>30.0%.结论 阴沟肠杆菌耐药现象严重,美罗培南、亚胺培南、阿米卡星、头孢吡肟为治疗阴沟肠杆菌感染的有效药物,合理选择抗菌药物要以药敏结果为依据.

Abstract:Objective To investigate epidemic and drug resistance analysis of Enterobacter cloacae in lower respiratory infections to provide evidence for clinical treatment.Methods We analyzed the sputum samples from the patient in Jan,2008-Dec,2010 by the Vitek bacteria identification and drug resistance analysis system.The results were analyzed statistically.Results The proportion of Enterobacter cloacae in hospital were 2.5%,2.4%,1.5% in 2008,2009,2010 respectively.The detection rate of Enterobacter cloacae in Neurosurgery department and urinary of internal medicine department were higher than the average in the hospital.There were no drug resistant stain to Imipenem and Meropenem in the study.The drug resistant rate of Amikacin and Cefepime were 14.0% and 27.1% respectively,the drug resistant rate of other antibiotics were greater than 30.0%.Conclusion The drug resistant status of Enterobacter cloacae were so serious that we must make reasonable choose on the basis of result of the drug sensitive test.Imipenem and Meropenem Amikacin and Cefepime were effective drugs to treat the infections of Enterobacter cloacae.
