腹腔镜及开腹胆囊切除术的临床比较分析

目的总结和探讨腹腔镜及开腹胆囊切除术的手术适应证、并发症及疗效。方法将我院2008年7月至2011年2月收治的100例患者作为本次的研究对象,对照组即开腹胆囊切除术组,本组共48例;实验组即腹腔镜胆囊切除术组,本组共52例,观察两组患者的并发症及疗效。结果对照组手术时间(98.6±12.5)min,出血量(123.5±20.1)ml,排气时间(31.6±4.6)h,VAS疼痛评分(6.5±2.1)分,并发症发生率18.8%,其中4例出现胆囊破口,2例出现胆道损伤,3例出现胆汁外溢。实验组手术时间(60.3±10.2)min,出血量(74.9±17.2)ml,排气时间(18.5±3.5)h,VAS疼痛评分(3.5±1.9)分,并发症发生率3.8%,其中1例出现胆囊破口,1例出现胆道损伤。结论腹腔镜胆囊切除术手术创伤小、并发症小、疗效好、恢复快、对患者的免疫功能影响较小等优点,相对于开腹胆囊切除术综合优势多,适用于临床推广使用。

Clinical comparative analysis of Laparoscopic and Open cholecystectomy

Objective To summarize and discuss the laparoscopic and open cholecystectomy on operation indications, complications and curative effect. Methods 100 patients from July 2008 to February 2011 as the research object, the control group was open cholecystectomy group, this group includes 48 cases,the experimental group that laparoscopic cholecystectomy group, this group includes 52 cases, were observed in two groups of patients with complications and curative effect. Results In the control group, operation time （98.6±12.5） min, bleeding volume （123.5±20.1） ml, exhaust time （31.6±4.6） h, VAS pain score （6.5±2.1）, the incidence of complications was 18.8%, including 4 cases of gallbladder rupture,2 case of billary duct injury, 3 cases of bile leakage.In the experimental group, operation time （60.3±10.2） min, bleeding volume （74.9±17.2） ml, exhaust time （18.5±3.5） h, VAS pain score （3.5±1.9）, the incidence of complications was 3.8%, including 1 cases of gallbladder rupture, 1 cases of billary duct injury.Conclusion Laparoscopic cholecystectomy operation trauma, complication of small, good curative effect, quick recovery, on the immune function of patients with affected the advantages of smaller, compared to open cholecystectomy comprehensive advantages, applicable to clinical use.
