纤维支气管镜在机械通气患者中的临床应用

目的评价纤维支气管镜（纤支镜）在机械通气患者中的应用价值。方法在监护下对107例机械通气患者行纤支镜（OlympusP60）检查及治疗。结果对血氧饱和度下降（SaO2）、呼吸和心率不稳定、气道压力明显不稳定、血痰、肺不张、严重肺部感染等能查找出原因，并予相应的镜下治疗，效果显著；纤支镜引导经鼻气管插管、换管及引导插胃管成功率高、效果好。结论纤支镜在机械通气患者中的应用可以发挥重要作用。

Application of fiber bronchoscope to mechanical ventilation

Objective To investigate the value of applying fiber bronchoscope to the patients with mechanical ventilation. Methods Undermonitoring, 107 patients with mechanical ventilati on were examined and treated with fiber bronchoscape （ Olympus P60）. Results The reasons, such as oxygen saturation decline, irregularity of respiration and heart beat, unstability of airway pressure, bloody sputum, atelectasis and serious pul - monary infection, could be found out, and then the corresponding fiber bronchoscope treatment was performed with good results. There was higher success rate and better effect during pernasal tracheal intubation, tube exchange and nasogastric intubati on guided by fiber bronchoscope. Conclusion Fiber bronchoscope can play an important role in the applicati on to mechanical ventilation.
