术前栓塞联合手术切除颅后窝实质性血管母细胞瘤的疗效分析

摘    要:目的 探讨术前栓塞联合手术切除颅后窝实质性血管母细胞瘤的疗效。方法 回顾性分析2011年1月至2021年6月手术治疗的56例颅后窝实质性血管母细胞瘤的临床资料。结果 56例中,21例(观察组)先行血管内栓塞治疗,再行开颅肿瘤切除术;35例(对照组)直接行开颅肿瘤切除术。与对照组相比,观察组肿瘤切除时间明显缩短(P<0.05),术中出血量明显减少(P<0.05),肿瘤全切除率明显提高(P<0.05),术后3个月KPS评分明显改善(P<0.05)。结论 颅后窝实质性血管母细胞瘤血供丰富,先行血管内栓塞处理肿瘤供血动脉,可有效减少术中出血,提高肿瘤全切率,改善病人预后。

Abstract:Objective To investigate the clinical efficacy of preoperative embolization combined with microsurgery for the patients with posterior cranial fossa solid hemangioblastoma. Methods The clinical data of 56 patients with posterior cranial fossa solid hemangioblastoma who underwent surgery from January 2011 to June 2021 were retrospectively analyzed. Results Of 56 patients, 21 patients (observation group) were treated with preoperative embolization combined with craniotomy, and 35 patients (control group) underwent craniotomy. Compared with the control group, the tumor resection time significantly decreased (P<0.05), the intraoperative blood loss significantly reduced (P<0.05), the total tumor resection rate significantly increased (P<0.05), and the KPS score significantly improved at 3 months after operation (P<0.05) in the observation group. Conclusions Abundant blood supply is common in the posterior cranial fossa solid hemangioblastomas. For the patients with posterior cranial fossa solid hemangioblastoma undergoing microsurgery, preoperative embolization can effectively reduce intraoperative bleeding, increase the rate of total resection of tumor and improve the patients' prognosis.
