TSGF、CEA、CA199联合检测在大肠癌诊断及预后评价的价值

目的探讨肿瘤特异性生长因子(TSGF)、癌胚抗原(CEA)、癌抗原199(CAl99)联合检测在大肠癌诊断及预后评价中的价值。方法选择大肠恶性肿瘤患者93例,空腹静脉采血。TSGF采用生化比色定量法测定,CEA、CA199用电化学发光免疫法测定。全部数据用SPSS12·0软件处理。结果TSGF、CEA和CA199的敏感性分别为40·86%、35·48%和16·13%。三者相关分析发现TSGF与CEA之间存在正相关(P<0·05)。TSGF联合CEA检测敏感性为59·14%,TSGF联合CA199检测敏感性为48·39%,CEA联合CA199检测敏感性为45·16%,三者联合检测的敏感性为64·52%,与单两者联合检测有统计学差别(P<0·05)。TSGF、CEA、CAl99与临床病理学之间关系TSGF的表达随淋巴结转移或远处转移而增强,差别有显著性意义(P<0·05)。CEA的表达随Dukes分期的升高或淋巴结转移而增高,差别亦有显著性(P<0·05)。发现CAl99的表达随Dukes分期的升高或远处转移而增高,差别亦有统计学意义(P<0·05)。TSCF的表达与CEA成正相关(P=0·01),与CA199不相关(P=0·14),而CEA与CA199之间亦无相关性(P=0·65)。结论①TSGF在大肠癌的早期诊断中有重要意义,其敏感性较CEA、CA199高。②联合检测TSGF、CEA及CAl99的表达不仅可以提高大肠癌的检测率,而且对大肠癌病理分期、预后判断等都有十分重要的意义。

The diagnostic and prognostic values of joint determination of TSGF, CEA and CA199 on patients with colorectal carcinoma

Objective To probe the diagnostic and prognostic value of the joint detection of tumor specific growth factor (TSGF), Carcinoembryonic antigen(CEA) and cancer antigen(CA199) in patients with colorectal carcinoma.Methods 93 patients with colorectal carcinoma were chosen and 5ml of veinous blood was drawn from fasting persons in the early morning. The serum level of TSGF was determined by biochemical colorimetry. The serum level of CEA and CA199 was determined by Eelectrochemoluminescence. All data were analyzed by SPSS12.0 software. Results The sensitivity of TSGF,CEA and CA199 was 40.86%,35.48% and 16.13% respectively. The combination of any two markers had higher sensitivity than the use of single marker, such as the sensitivity of combination of CEA and TSGF was 59.14%, and the sensitivity of combined marker of three was 64.52%.There was significant correlation between expression of serum TSGF and lymph node metastasis and distant metastasis. There was also significant correlation between expression of CEA and lymph node metastases and Dukes' staging. So did CA199 with Dukes' staging and the distant metastases. There was significant correlation between the expression of TSGF and CEA (P=0.01). Conclusion Combined determination of three markers could significantly enhance the sensitivity and valuable for diagnosis and prognosis of colorectal carcinoma.
