金属对金属大头径股骨头假体全髋关节置换术的临床应用

目的探讨大头径全髋关节置换术(THA)的临床应用及优势和缺陷。方法自2006年10月至2007年6月,对22例患者26个髋关节进行大头径THA手术,其中男15例,女7例;年龄32～51岁,平均44岁;左髋7例,右髋11例,双髋同时进行置换4例;术后平均随访时间27.5个月。术后评估的指标有:术后髋关节脱位率,翻修率,感染率,患者满意度调查、髋关节功能检查、X线和Harris评分分析。结果术后患者感染率,翻修率,脱位率均为"0"。所有患者均未行翻修手术。X线显示:所有大头径全髋关节假体植入位置满意,无假体松动、移位现象出现。问卷调查显示:患者对18个髋关节的术后情况表示满意。Harris评分平均指数从术前的41(31～50)分提高到术后的92(82～96)分。结论大头径THA是治疗髋关节疾病的一种有效方法,早期随访结果较好,在稳定性、活动度等方面有较好的优势。

Clinical application of large diameter femoral head in uncemented total hip arthroplasty

Objective To investigate the application of large diameter head in total hip arthroplasty,and to demonstrate if this will achieve better clinical outcomes.Methods From Oct 2006 to Jun 2007,22 patients were enrolled in department of Orthopedics for different reasons.Fifteen were male and seven were female.The patients＇ mean age was 44 years,ranging from 32 to 51 years.Seven patients were performed THA on the left,and eleven patients were performed THA on the right.Furthermore,there were four patients treated by THA on bothe sides.Patients were evaluated by self assessment,hip function examination,radiographs and Harris hip scores postoperatively.The average follow-up time was 27.5 months.Results No dislocation was revealed and no patient received revision.No complication of infection occurred.Radiographic outcomes showed that all implants were in perfect position.The results of self assessment showed that patients were satisfied with 18 hips.The Harris scores were preoperative 41 （ranging from 31 to 50）and postoperative 92 （82 to 96）.Conclusions Large-diameter femoral head arthroplasty of the hip is a useful treatment to relieve pain and restore the hip function of patients.
