滋阴利水法治疗老年糖尿病肾病临床观察

目的探讨滋阴利水法治疗老年糖尿病肾病（DN）患者的临床疗效。方法将120例老年DN患者随机分为治疗组和对照组,治疗组在常规治疗方案的基础上加用滋阴利水为主的中药降糖保肾冲剂,疗程为4周,观察两组临床疗效、中医证候疗效、治疗前后尿微量白蛋白（UAER）、α1-微球蛋白（α1-MG）、24h尿白蛋白排出量变化及血脂各项指标。结果治疗组临床疗效总有效率85.6%,明显优于对照组66.7%（P〈0.05）;治疗组中医证候分级疗效总有效率90.0%,明显优于对照组56.7%（P〈0.01）;治疗组治疗前后UAER、α1-MG、24h尿白蛋白排出量明显优于对照组（P〈0.01）。结论滋阴利水法可有效治疗老年DN患者,或延缓其向终末期肾病发展,且无不良反应,提高老年患者的生活质量。

Clinical Observation on Elderly Patients with Diabetic Nephropathy Treated with Nourishing Yin and Clearing Damp Therapy

Objective To investigate the clinical efficacy of the nourishing Yin and clearing damp therapy for elderly patients（pts）with diabetic nephropathy（DN）.Methods One hundred and twenty elderly pts with DN were randomly divided into two groups：Control group treated with routine treatment and the treatment group treated with routine treatment and Jiangtangbaoshen granule for 4 weeks.The clinical efficacy and the levels of urinary albumin excretion rate（UAER）,α1-microglobulin（α1-MG）,24 hour urine albumin and blood lipid were observed.Results Total clinical efficacy（85.6%）in treatment group was higher than that in control group（66.7%,P0.05）.Total efficacy（90.0%）in TCM symptom was higher than that in control group（56.7%,P0.01）.The levels of UAER,α1-MG and 24 hour urine albumin in treatment group was lower than that in control group（P0.01）.The levels of total cholesterol（TC）in treatment group was lower than that in control group（P0.01）.Conclusion The nourishing Yin and clearing damp therapy was effective for treatment of elderly pts with diabetic nephropathy.
