冠心病介入治疗围术期中医证候特征及分布规律的研究

目的探索冠心病介入治疗围术期中医证候特征及分布规律,了解介入治疗对中医证型的影响。方法选择经冠状动脉造影确诊的冠心病患者405例,分析介入治疗围术期中医证候及分布规律,四诊指标变化情况,证型相兼、虚实的变化规律。结果冠脉介入治疗后气虚证增加,血瘀证、寒凝证减少;介入治疗后胸痛、胸闷、脉涩等均显著改善,神疲乏力、纳食减少、脉细等显著性增加;术前实证所占比例最大,术后本虚标实证所占比例最大。结论介入术对冠心病辨证分型的影响为介入治疗后气虚证增加,血瘀证、寒凝证减少,提示应充分关注介入治疗后补气活血、标本兼治的重要性和必要性。

Study on Chinese Medicine Syndromes Characteristics and Distribution Rule Around Percutaneous Coronary Intervention Perioperative Period

Objective To explore the characteristics and distribution rule of Chinese medicine syndromes at perioperative period of percutaneous coronary intervention（PCI）,and influence of syndrome on account of PCI.Methods Four hundreds and five patients（pts） with coronary heart disease（CHD）were selected for the study of coronary angiography,characteristics and distribution rule of Chinese medicine syndromes.The rule of Chinese medicine syndromes complexes,deficiency and excess syndrome at PCI perioperative period were analyzed.Results Qi deficiency syndrome was increased,blood stasis syndrome and cold coagulation syndrome were decreased after PCI.Chest pain,chest distress,hesitant pulse were decreased significantly.Spiritlessness,weakness,anorexia,and thready pulse were increased significantly after PCI.Excess syndrome had a biggest proportion before PCI.Deficiency in root and excess in superficiality had a biggest proportion after PCI.Conclusion PCI had influence for Chinese medicine syndromes of CHD.Qi deficiency syndrome were increased,blood stasis syndrome and cold coagulation syndrome were decreased after PCI.The results suggested that more attention should be paid to tonifying Qi,activating blood circulation,and treating both Biao and Ben syndrome after PCI treatment.
