脑卒中患者医院社区防治一体化护理管理模式的研究

目的探讨脑卒中患者医院社区防治一体化护理管理模式，为脑卒中患者的护理管理提供科学的指导依据。方法选择符合标准的脑卒中住院患者64例，随机分成观察组和对照组各32例，观察组实行一体化管理模式，对照组单纯实行住院护理管理。结果观察组在疾病相关知识掌握率、对护理工作满意度方面均优于对照组，差异有统计学意义（P〈0．01）。结论实行脑卒中患者医院社区防治一体化有助于提高患者及家属对疾病的认识，提高治疗的依从性。提高脑卒中患者生活质量。

Stroke Prevention in Patients with Community-based Care Management Model of the Integration of Research

Objective To investigate the prevention and treatment of stroke in patients with community - based integrated care management model of care for stroke patients to provide scientific guidance to management based on type. Conclusion Implement community - based prevention and treatment of stroke in patients with the integration of hospital patients and their families help to improve understanding of disease, improve treatment compliance and improve the quality of life of stroke patients.
