凝血功能与肺癌分期及病理分型的关系

目的 研究肺癌患者的凝血功能与肿瘤病理类型及分期的关系，并探讨其临床意义。方法 收集南京医科大学第一附属医院胸心外科2012年1月至2012年6月连续76例行手术治疗的肺癌患者，检测患者血浆组织因子（TF）、D-二聚体、血浆纤维蛋白原（FIB）、纤维蛋白原降解产物（FDP）水平及血小板（PLT）计数，按不同病理类型及术后分期对以上指标进行比较。结果 Ⅲ期、Ⅳ期患者的血浆D-二聚体、TF水平明显高于Ⅱ期患者，差异有统计学意义（P＜0.05） 。肺腺癌患者TF、D-二聚体水平明显高于肺鳞癌患者，差异有统计学意义（P＜0.05）。结论 肺癌患者凝血功能与病理类型、肿瘤分期有关，早期发现并改善高凝状态，可能对肺癌预后有意义。

The association between coagulation function and pathological types and stages in lung cancer patients

Objective To study the association between coagulation function and pathological types and stages in patients with lung cancer.Methods A total of 76 patients from the Department of Cardiothoracic Surgery of the First Affiliated Hospital of Nanjing Medical University from January 2012 to June 2012 were enrolled in the study.The levels of plasma tissue factor（TF）,D-dimer,plasma fibrinogen（FIB）,fibrinogen degradation product（FDP） and platelet couts were detected and compared in patients of different clinical stages and pathological types.Results The plasma TF,D-dimer levels were significantly higher in patients with stage Ⅲ,Ⅳ than those in stage Ⅱ （P 〈 0.05）.The TF,D-dimer levels were higher in adenocarcinoma than those in squamous cell carcinoma （P 〈 0.05）.Conclusion Patients with lung cancer are in high hypercoagulable state,which are significantly relevant with clinical stages and pathological types of malignant tumors,it may be served as prognostic marker for lung cancer.
