尿淀粉酶与尿肌酐的比值诊断急性胰腺炎的评价

目的:探讨尿淀粉酶/尿肌酐(uamy/Ucr)测定在急性胰腺炎(AP)诊断中的临床诊断价值。方法:对40例AP患者、39例非AP患者和43例健康对照组人员进行血清淀粉酶(Samy)、尿淀粉酶(Uamy)及尿肌酐(Ucr)测定。结果:AP Uamy/Ucr的诊断特异性最高,其灵敏度和特异性分别为90.0%和84.6%,而临床常用的诊断指标Samy、Ua- my的灵敏度和特异性均低于前者。结论:Uamy/Ucr是较好的早期诊断AP的指标。

The Clinical Value of the Ratio of Urine Amylase to Urine Creatinine Assay in the Diagnosis of Acute Pancreatitis

Objective To assess the clinical value of the ratio of urine amylase to urine creatinine in the diagnosis of acute pancreatitis.Methods Serum amylase,urine amylase and urine creatinine in 40 patients with acute pancreatitis,39 patients with nonpancreatitis acute abdomen and 43 healthy controlled people were detected and compared.Results The ratio of urine amylase to urine creatinine had a high sensitivity and specificity of 90.0% and 84.6%,respectively( cut off value,74 U per mmol).The sensitivity and specificity of the serum amylase assay and the urinary amylase assay were lower than the aforemen- tioned indexes.Conclusion The ratio of urine amylase to urine creatinine assay is useful and better than the traditional assay for the diagnosis of acute pancreatitis in the early stage.
