| 摘 要： | 目的探讨和总结脾损伤保脾治疗的方法和经验。方法回顾性分析1995年1月至2004年6月本院保留脾脏治疗32例脾外伤的临床资料及其治疗结果。结果32例脾外伤中,19例入院时血液动力学稳定,或输血后稳定行非手术治疗,其中17例(占89%)治愈(2例因迟发脾破裂而保脾治疗失败);15例行保留脾脏或脾脏组织手术,均临床治愈。结论严格掌握适应证,非手术治疗和保留脾脏手术是治疗脾外伤的一种安全有效的手段。

| Abstract: | Objective:To explore the experiences of the management of splenic injuries with splenic preservation. Methods:From January 1995 to June 2004, 32 consecutive patients with splenic trauma were managed by splenic preservation methods in our hospital. The clinical date and results of treatment of these patients were reviewed retrospectively . Results:Among 32 cases of splenic injuries, 19 were hemodynamically stable on admition, or stable after blood transfusion. Out of which 17 of them (89%) were managed successfully by non-operative(2 of them caused by a delayed bleeding were failed); 15 cases underwent spleen-preserving or splenic-tissue-conserving surgery. All of them were healed. Conclusion:Splenic salvage by means of nonoperative management or spleen-preserving operation is feasible and safe according to the indications seriously. |
