股前外侧皮瓣血管蒂的变异及处理

目的探索股前外侧皮瓣血管蒂的变异及处理。方法在５２块股前外侧皮瓣切取移植手术中，详细观察该皮瓣血管蒂的有关解剖。结果发现血管蒂起始于旋股外侧动脉降支系统４４块、横支５块、股深动脉２块、股动脉１块。１２块为肌间隙皮支，４０块为肌皮支。结论股前外侧皮瓣血管蒂主要起始于旋股外侧动脉降支系统，但也存在一些变异。先作皮瓣内侧切口，仔细解剖，寻找血管蒂，根据血管蒂进皮点的高低，作相应切口调整，皮瓣往往能成功切取。

Vascular variation of the anterolateral thigh flap and its management.

Objective To study the vascular variation of the anterolateral thigh flap and its management.Method Anatomy of the vascular pedicle was carefully observed on the transfer of 52 free anterolateral thigh flaps.Result The vascular pedicles were derived from the descending branch of the lateral circumflex femoral system(44),the transverse branch of that system(5),the profunda femoris artery(2),or the femoris artery(1).Twelve flaps are the septocutaneous perforators and forty the musculocutaneous perforators. Conclusions The vascular pedicles of the anterolateral thigh flaps are mainly derived from the descending branch of the circumflex femoral system,but there are some vascular variations.The pedicle should be explored carefully after a medial incision of the flap was made.The incision should be readjusted according to the entry location of the cutaneous perforator so that the flaps may be obtained easily.
