知情选择中提供IUD种类与效果关系的分析

目的:探索避孕方法知情选择工作中供选择的宫内节育器(IUD)种类数量与其使用效果间的关系。方法:对9省农村18922例放置IUD妇女的流行病学调查数据进行聚类分析,并进行生存分析和Cox回归分析。结果:IUD种类数量与其使用效果并非线性关系。比较IUD使用失败率,使用IUD>5种的地方(14.31%)明显高于使用IUD≤5种的地方(9.89%)。进一步分层分析和多因素Cox回归分析发现,使用IUD>5种的地方发生使用失败结局的可能性高于使用IUD≤5种的地方。结论:使用IUD的种类数量过多,IUD的使用效果未必好。为了保证IUD的使用效果,应对所提供IUD种类进行合理的选择。

Analysis of Relation between the Number of IUD Types and Their Use Effectiveness

Objective: To explore the relation between the numbers of IUD type provided in informed choice work and their use effectiveness. Methods: Cluster analysis was applied in examining data from a population-based epidemiological study which was investigation and follow-up of 18,922 IUD users in rural areas of nine province. Other statistical methods such as survival analysis and Cox regression were used to supplement the whole analysis. Results: Non-linear correlation was found between the number of IUD types and their use effectiveness. IUD failure rate in places where more than 5 types of IUDs were provided for informed choice (14.31%) which showed much higher than that in places where 5 or below types of IUDs were offered (9.89%). Further stratified analysis and multi-factor Cox regression analysis showed that places with more than 5 types of IUDs provided were more likely to have IUD use failure cases than those with 5 or below types of IUDs provided. Conclusion: To provide more types of IUDs do not appear an encouraging prospect of IUD use. Types of IUDs need to be rationally selected.
