严重髋臼骨折

严重髋臼骨折是一种复杂损伤。可分为前、后柱、横、Y／T形和臼顶骨折五类。大多数髋臼骨折均有错位和股骨头中心脱位。牵引不能复位者应行手术治疗。本文报告17例严重髓臼骨折，其中9例行手术治疗效果满意。

Severe Fractures oe the A.cetabulum

Fractures of the acetabulum usually follow severe injuries. They are usually associated with displacement of bony fragmeats and dislocation of femoral head. Severe fractures of the acetabulum may be divided into 5 types, 1. fracture of the anterior column; 2. fracture of the pasteriot column; 3. transverse fracture; 4. Y- shaped or T-shaped fracture; 5. fracture of the dome of acetabular fossa. Surgical treatment is recommended wheu closed reduction fails. Basing on our clinical experience jncluding 9 cases operated upon and the review of literature in relevance, we have used 4 different surgical approaches in the treatment of various types of severe fractures of the acetabulum; 1. extended Smith-Petersoaincisioa for fractures of the anterior column and the dome; 2. Posterilateral incision for fractures of the posterior column and the domes 3. C-shaped incision designed by the authors or modified Ollier incision for transverse fractures and for Y-shaped or T-shaped fractures. Seventeen cases of severe fractures of the acetabulum, among which 9 cases operated upon, were treated with good clinical results and are here reported.
