无痛人工流产术中异丙酚不同输注方式的比较

目的:观察无痛人工流产术中采用不同方式输注异丙酚的临床效果,探讨人工流产术最佳麻醉方法.方法:120例ASA I级接受无痛人流术的病人,随机分为两组.异丙酚恒速输注组(C组)及异丙酚靶控静脉输注组(T组),每组60例.C组:异丙酚恒速输注速率为每小时6mg.kg-1;T组:异丙酚的血浆靶浓度为6mg/L.异丙酚诱导,待病人意识消失(睫毛反射消失)后开始手术.扩张宫颈结束时停止给药.观察患者麻醉清醒时间、准确定向时间、异丙酚总用量、呼吸抑制评分及麻醉效果等指标.结果:两组麻醉清醒时间分别为(5.6±1.3)min及(5.3±1.4)min、准确定向时间分别为(6.7±1.4)min及(6.3±1.5)min,组间比较无统计学差异(P>0.05).两组麻醉效果优良率分别为90%及82.4%,组间比较无统计学差异(X2=0,P>0.05);T组异丙酚总用量及呼吸抑制评分明显小于C组(P<0.01),但无临床意义.结论:采用恒速输注泵给予异丙酚,其临床效果与采用靶控输注泵给药类似.无论采用何种方式输注异丙酚,都必须加强麻醉期间的呼吸管理.

Observation of propofol given by different infusion mode for analgesic artificial abortion

Objective To observe the effects of propofol with different infusion mode for analgesic artificial abortion.Methods 120 patients of ASA I undergoing analgesic artificial abortion were randomly divided into two groups,60 patients in each gmup:Group C and Group T.The patients in C group were given pmpofol at a plasma concentration of 6 mg/L;the plasma concentration of ptopofol was set at 3 mg/L in group T.The operations were started after the patients lost consciousness(no response to voice command and loss of eyelash reflex)and infusion bump were stopped when dilating cervix uteri Was over.Analepsia time,definite directional time.total dosage of propofol,anaesthesia effect fineness rate,respiratory depression score were observed.Results Analepsia time in group C and group T were(5.6±1.3)min and(5.3±1.4)min,and definite directional time in two group were(6.7±1.4)min and(6.3±1.5)min,anaesthesia effect fineness rate were 90%and 82.4%there were no difference between the two groups(P>0.05).The total dosage of propofol and score Of respiratory depression in group T Was significantly lower than that in group C(P<0.01),but there were no clinical value.Conclusion Pmpofol with constant speed infusion showed a likely anesthetic effect compared with the TCI of propofol on analgesic artificial abortion,and attention should be paid to its respiratory depression effect.
