咪唑安定诱导和普鲁卡因静脉复合麻醉对颅内压及脑灌注压的影响

摘    要:研究比较异丙酚与咪唑安定在高颅内压患者的麻醉诱导,并用普鲁卡因静脉复合维持麻醉,对平均动脉压(MAP),颅内压(ICP),脑灌注压(CPP),心率(HR)的影响。26例ASAI~Ⅱ级颅内肿瘤切除术患者依次序为异丙酚组(P组,n=10),咪唑安定组(M组,n=16)。结果表明异丙酚和咪唑安定在诱导中都有一过性循环功能抑制,但能维持脑的自身调节功能,有明显降低颅内压作用(P〉0.05或P〈0.01)P

Influences of Midiazolam Induction Combined With Intravenous Procaine Anesthesia on Intracranial Pressure and Cerebral Perfusion Pressure

Abstract:To compare the influences of anesthesia induced with midiazolam combined with intravenous procaine on the mean arterial pressure (MAP) . intracranial pressure (ICP),cerebral perfusion pressure (CPP)and heart rate (HR) of patients with intracranial hypertension, with those induced with propofol,twenty-six adult patients with intracranal tumors ASA grade l-11,underwent neurosurgery ,were divided into two groups according to order of operation:propofol indution group (n = 10). and midiazolam induction group (n=16). The results showed that both propofol and midiazolam had a temporary circulation inhibltion during anesthesia indution stage, but cerebral autoregulation functions could be maintained and intracranial pressure could be decreased considerably(P <0. 05 or P <0.01). During operation period,the heart rate increased and the intracranial pressure decreased gradually in the propofol group,but in the midiazolam group,the heart rate and mean arterial prcssure remained stable and the intracrania1 pressure decreased remarkably. The results suggest that midiazolam anesthesia induction can decrease intracranial pressure contineously and maintain cerebral autoregulation functions and satisficatory anesthesia depth,which plays a good protective role in the brain functions during neurosurgery.
