系统性红斑狼疮中枢神经系统感染与狼疮性脑病临床分析

目的:探讨系统性红斑狼疮(SLE)合并中枢神经系统感染的临床特点,分析其和神经精神狼疮的鉴别。方法:回顾分析资料完整的16例SLE合并中枢神经系统的病例资料,并与12例SLE合并神经精神性狼疮患者进行对比分析。结果:中枢神经系统感染组发病时激素用量大,狼疮疾病活动指数(SLEDAI)评分明显低于神经精神狼疮组;临床表现以发热、头痛为主;脑脊液压力、细胞数和蛋白质高于神经精神狼疮组。结论:SLE中枢神经系统感染临床表现复杂,可以从临床表现、活动指数及脑脊液改变等方面进行鉴别。

Clinical analysis of systemic lupus erythematosus complicated with central nervous system infection and neuropsychiatric lupus

Objective Research and analysis the clinical manifestations of systemic lupus erythematosus complicated with central nerv- ous system infection and neuropsychiatrie lupus. Method Through the retrospective analysis of e]Anical data of the 16 cases of patients with SLE complieated with central nervous system infection （infection group）, and 12 patients with neuropsyehiatrie lupus at the same period served as control group. Results In infection group,the dosage of prednlsone was higher,and lupus disease activity index was significantly lower than in control group. The main clinical manifestations in infection group were fever,headache. There was significant difirerenee in ee- rebrospinal fluid（CSF） pressure ,cell eotmt and protein between infection group and control group. Conclusion Central nervous system in- fection could be distinguished with neuropsyehiatrie lupus through clinical malnifestation,disease activity index,and changes in CSF.
