某三级甲等医院抗菌药物临床应用专项整治活动效果分析

目的:规范医院临床抗菌药物合理使用。方法:某三级甲等医院根据卫生部《抗菌药物临床应用管理办法》(卫生部令第84号)指导原则开展抗菌药物临床应用专项整治活动。分析确定医院应用抗菌药物存在的主要问题,参照国内外有关文献,制订"医院抗菌药物临床应用实施细则"、"医院手术预防应用抗菌药物管理规定","抗菌药物分级管理目",等抗菌药物合理应用管理文件。依据以上文件与规定,采取一系列干预措施。数据采集为医院东软HIS5.0系统,从中选取的300例作为干预组进行抗菌药物是否合理应用调查。利用病历检查和从医院信息系统提取数据的方法,对住院患者抗菌药物使用情况,介入手术预防用药指标和围手术期预防用药指标进行分析。结果:干预前围手术期预防性应用抗菌药物存在的主要问题,如抗菌药物品种选择不当,给药时机不当和用药疗程过长等,在干预后均有显著改善。结论:在医院内开展抗菌药物临床应用专项整治活动后取得明显成效。对促进临床一线安全、有效、经济,合理地使用抗菌药物起到了积极的促进作用,值得推广借鉴。

Analysis of the effect of clinical application of antibacterial drugs special rectification activities

Objective To standardize the clinical rational use of antibacterials.Method In our hospital,according to guiding principles of the Ministry of Health＂ antibacterial drug clinical application management approach ＂（Ministry of health,order No.eighty-fourth）,we developed special rectification activities on clinical application of antibacterial drugs.We analyzed the main problem of using the antibacterial drugs in our hospital,referring to the concerning documents,we evolved the＂hospital clinical application of detailed rules for the implementation of antibacterial drugs＂＂Hospital surgery preventive application of antimicrobial agents management regulations＂,＂antimicrobial drugs classification management＂,file management of reasonable application of antibacterial drugs.According to the file with the specified above,a series of intervention measures were taken.Data collection was done by the hospital neusoft HIS5.0 system,We chose 300 cases of hospitalized patients from June 2012 to July 2012 as intervention group to investigate whether antibacterial drugs were reasonably applied.Using medical record and methods of extracting data from the hospital information system,the use of antibacterial agents in hospitalized patients,interventional surgery and perioperative prophylaxis were analyzed.Results Before the intervention,there was the main problems in the prophylactic use of antimicrobial agents,such as improper antimicrobial species selection,improper dosing time and the use of pharmacologic too long etc.,and it was significantly improved after the intervention.Conclusion The special rectification activities of clinical use of antibiotics in hospitals achieve significant results.It plays a positive role in promoting clinical first-line security,effective,economic,and rational use of antimicrobial drugs.It is worthy of promoting.
