MMP-1、TIMP-1和TNF-α在溃疡性结肠炎中表达的意义

目的研究基质金属蛋白酶-1（MMP-1）及其抑制剂-1（TIMP-1）、肿瘤坏死因子-α（TNF-α）在溃疡性结肠炎（UC）结肠黏膜不同区域以及不同病情中的表达，探讨它们在UC发生发展中的作用和I临床意义。方法收集UC不同区域的结肠黏膜组织，分为轻、中、重三组。采用逆转录-聚合酶链反应（RT—PCR）及免疫组化检测三种因子的表达。结果MMP—1、TIMP—1、TNF-α在溃疡区的表达高于非溃疡区（P〈0．05）。MMP-1、TNF-α及TIMP-1在重型中的表达高于轻型（P〈0．05）。MMP-1／TIMP-1、TNF-α与病情严重程度相关（P〈0．05）。结论结肠黏膜中TNF-α表达升高，引起黏膜组织免疫失调，可能引起MMP-1与TIMP-1表达失衡，在UC的发生发展中起重要作用。MMP-1／TIMP-1和TNF-α可作为评价UC病情的指标。

Expressions of MMP-1, TIMP-1 and TNF-a in ulcerative colitis

Objective To examine the expression of matrix metalloproteinase-1 （MMP-1） , tissue inhibitor of metalloproteinase-1 （TIMP-1）and tumor necrosis factor-α （TNF-α） in colon mucosa of patients with ulcerative colitis （UC）, and evaluate their clinical significance and roles in UC pathogenesis. Methods The biopsy specimens were taken from different areas and were classified into mild, moderate and severe groups. RT-PCR and immunohistochemistry were used to examine the expressions of the three growth factors. Results MMP-1, TIMP-1 and TNF-α in ulcerated areas were significantly higher than those in non-inflamed areas （P 〈 0.05）. MMP-1 , TIMP-1 and TNF-α in severe groups were significantly higher than those in mild group （P 〈0.05）. MMP-1/TIMP-1 and TNF-α were correlated with the severity of clinical symptoms （P 〈 0.05 ）. Conclusion The high expression of TNF-α may cause the immune disturbance of colonic mucosa, leading to the imbalance between MMP-1 and TIMP-1, which plays an important role in the development of UC. MMP-1/TIMP-1 and TNF-α could be used to evaluate the clinical severity of UC patients.
